The Toolkit for Community Action was developed by the National Partnership for Action to End Health Disparities to help individuals, organzations and policy makers raise awareness about health disparities, engage others in conversations about the problems and solutions, and provide information and tools to help us address health in our communities. The Toolkit includes background information, quick facts, letter to the editor templates, five steps you can take to address health disparities, and many links to additional resources.
